The Department of Aging and Community Services supports older adults, adults with disabilities, family caregivers, and residents in long-term care facilities. Its services help older and disabled individuals live as independently as possible in the community, promote healthy aging and community involvement, and assist family members in their vital caregiving roles.
Funding for services related to older adults 60+ years is primarily from the Federal Older Americans Act ( Area Plan )

San Joaquin County Adult Protective Services Hotline
Adult Protective Services (APS) responds to confidential reports from anyone who suspects that an elderly person or an adult with a disability (dependent adult) who lives in Contra Costa County is being abused or neglected or is self-neglecting. Our 24-hour hotline is staffed by social workers who respond to these calls and will walk the caller through a series of questions to determine if a situation requires APS intervention. When a case is opened in APS, an APS social worker will be assigned to the case and will go out to the home of the alleged victim to determine what services, if any, are needed.

Ombudsman
The Ombudsman Program provides a number of critical services. Ombudsman act as the eyes and ears for residents in long-term care facilities such as nursing homes or board and care homes. Ombudsman encourage access to advocacy by informing residents of what kind of care to expect, by providing a mechanism to file a complaint, and by guiding residents through the process of advocating on their own behalf.
The Ombudsman Program investigates reports of abuse and neglect of seniors and dependent adults residing in long-term care facilities. There is no income eligibility.

Relief for Energy Assistance through Community Help (REACH)
We sponsor REACH, a one-time energy-assistance program that Dollar Energy Fund administers from 170 offices in Northern and Central California. REACH helps income-qualified customers who have experienced an uncontrollable or unforeseen hardship and are unable to pay for their energy needs.

Weatherization Program
The Weatherization Program can assist renters or owners by installing energy-saving measures and repairing homes, multi-unit dwellings, and mobile homes. These dwellings qualify for weatherization measures if the household income does not exceed a certain amount determined by the U.S. Department of Health and Human Services. Your residence may qualify for repair or replacement of a refrigerator, stove, water heater, or microwave; insulation; minor home repair, LED light bulbs; smoke and carbon monoxide detectors; shade screens; weather-stripping; and much more.
